Getting ready for your visit
In order for us to provide the best care for you, the following list will help you to prepare for your appointment.- Bring a valid photo ID and your Insurance Card.
- Call your insurance company to find out about co-pays and other coverage issues.
- Bring any and all X-rays, MRI’s, CT Scans from other groups or hospital visits.
- Bring medical records from prior visits, and or treatments that have been tried in the past, that you feel might be helpful.
- Bring a list of all medications you are taking, including any over the counter vitamins and supplements.
- Be honest and complete when talking with your doctor. Jot down any specific details about how and when your injury occurred and what you heard or felt.
- Print out and complete the new patient forms for the physician you will be seeing and follow instructions provided:

Insurance Accepted
Our office participates in most managed care health plans, however if you are unsure if we accept your health plan please
call 303-214-4500 to confirm this information. To view the list of accepted insurance plans. go to our
Patient Insurance page. While every effort is made to keep our affiliation with each company current, we cannot guarantee
your insurance coverage.
Payments
At the time of your visit you are responsible for your co-payment as determined by your individual
insurance carrier. Once your insurance company has been billed, it is your responsibility to pay any
deductibles, co-pays and all non-covered services.
Billing Questions
If you ever have any questions about your bill, please fill out our short, easy to use
billing inquiry form. We will attempt to respond to and resolve inquiries received through
our web-site within 48 business hours. However, some inquiries may require additional research to be resolved. We
do not offer responses via e-mail, since we cannot ensure the identity of who will receive our response and, therefore,
the confidentially of any information we may provide.
